Customer Instructions for
Lab Testing and Patient Portal

Thank you for ordering lab testing services!

​

T2YourHealth offers two avenues for lab testing services:

​

1) On-site testing at our clinical reference laboratory in El Paso, Texas.

Click here for more information about our patented next-generation T2 screening and diagnostic tests for metabolic health, wellness, performance, prevention, and longevity.

2) Nationwide testing in 47 U.S. states, except NY, NJ, RI.

Through our partnership with Ulta Lab Tests and Quest Diagnostics, we provide you with convenient, high-quality, low-cost testing services. This includes access to the 3,500+ tests in the Quest menu and 2,200+ Quest Patient Service centers across the U.S.

**These instructions are for Avenue 2: Nationwide Ulta/Quest testing.**

 

STEP-BY-STEP instructions on how to:

1) Download and save your itemized HSA/FSA receipt

2) Download your Quest Lab Requisition

3) Make a Quest PSC appointment

4) Prepare to visit the Quest PSC

5) Download your Quest lab test results

Step 1: Download and save your itemized HSA/FSA receipt 

​​

This detailed receipt is a required document for purchases made with a Health Savings Account (HSA) or Flexible Spending Account (FSA). To download, login to the patient portal at t2yourhealth.stratusdx.net/patientportal using your phone number or email. Click "My Results", then the "Requisition/Result Attachments" blue button at the far right. (The other two blue buttons are not used for Ulta/Quest orders.) Download and save the pdf document, entitled “Itemized Clinical Laboratory Receipt” with the T2YourHealth logo at the top. Check with your HSA/FSA program administrator to confirm eligibility for payment or reimbursement.

NOTE: Ulta/Quest lab services are not eligible for health insurance. Do not submit this receipt for reimbursement under any public/private insurance plan.

​

 

Step 2: Download your Quest Lab Requisition ("PSC HOLD ORDER")

​

The Quest lab requisition will be available in the patient portal within 2 business days after your order is placed. The lab requisition is the pre-paid “ticket” you must take to a Quest Patient Service Center (PSC) for blood/urine sample collection. To download your requisition, login to the patient portal as described in Step 1 and click the "Requisition/Result Attachments" blue button. The document shows “PSC HOLD ORDER” in orange font in the upper right corner.

Step 3: Make a Quest PSC Appointment

​

Go to appointment.questdiagnostics.com to select a Quest PSC location, date, and time for blood/urine collection. By default, the lab requisition shows the Quest location closest to your home address. However, you can visit any Quest PSC. If your tests require fasting overnight, please select an early morning time slot between 6 and 9 AM. Typically, the appointment takes about 15 minutes.

Step 4: Prepare to Visit the Quest PSC

​

At least two days prior to your appointment, read page 2 ("Patient Instructions") of the Quest lab requisition for important information on how to prepare for your appointment. If overnight fasting is required, do not eat or drink anything (except plain water) for 10-12 hours prior to your morning appointment. If you take AM medications, take them after your appointment has been completed. You can resume regular eating and drinking after your appointment is over.

IMPORTANT:  Bring a paper copy of page 1 of your lab requisition to your appointment. Quest will not accept a cell phone view of your requisition – a paper copy is required.

 

Step 5: Download your Quest lab test results

​

The results are released by Quest 1-3 business days after your appointment for most routine tests but may take longer for specialized tests. To view/download your Quest Report, log into the patient portal as described in Step 1 and click the "Requisition/Result Attachments" blue button at the far right. The report displays the green Quest logo in the upper left corner. You can also access your results from myquest.questdiagnostics.com by setting up a free account there.
